Becker's Spine Review

The 30 days that could make or break orthopedic episodesBy Sophie Eydis, Becker's Spine Review · July 2026

Becker's Spine Review turned to Dr. Shannon Carpenter for a deep dive on the 30-day post-discharge window that CMS's TEAM model has attached to every orthopedic episode. Her argument: the bundle is won or lost after the operating room, in disposition planning, rehab-facility oversight, and the follow-through that keeps patients from bouncing back to the ER, and hospitals that wait to build that muscle will be behind.

On what TEAM changed
“They're now holding the bag for a patient that they didn't choose.”
On preoperative optimization
“Preoperative optimization looks at the patient as a whole.”
“You're making sure that patient is a good candidate for the procedure, not just that they have the pathology of knee arthritis.”
On the biggest lever
“The biggest lever to pull is getting someone to go home instead of to a rehab facility.”
“You have to stay on top of the rehab facilities and be sure that there are clear and concrete goals.”
On the 30-day window
“The name of the game is really making sure people feel well taken care of so that they don't just pop back into the ER.”
“Most of the time, they just need reassurance.”
“You still have to have somebody at the end of that line to take those calls and call those patients and deal with the issues.”
On acting now
“Hospitals are large ships with small rudders.”
“I would want to be way far ahead of this.”
